We offer several packages of care based on the individuals needs, whether it be support based or more specialist care to take into consideration age, physical disability, older mental health or more complex mental health needs.

The care manager will carry out an initial assessment followed by a person centred care plan tailored to the individuals specific needs. Taking into account a holistic approach which identifies physical, psychological, social, emotional and spiritual needs. This ensures the individual receives all the care and support they need to live as independently as possible within the home. The care plan is written up with the resident and or family or if they wish an advocate.

Each resident is allocated a member of care staff to act as keyworker, the keyworker will work with them to ensure their care plans are monitored and reviewed regularly. The keyworker also provides regular 1:1 support whether it be for emotional support, or a shopping trip or an escort to visit the chiropodist
